You may ask for a copy of the complaint form found in the policy manual or simply write your complaint on a piece of paper. If you have difficulty in writing due to a disability or for any other reason, you may ask for assistance in preparing the written complaint. The managing attorney will investigate and review your complaint and will inform you of the result in writing.
